Understanding Low Back Pain
Introduction to Low Back Pain
Low back pain is a widespread issue, with research indicating that up to 90% of people in the UK will experience it at some point in their lives. This translates to millions of individuals dealing with discomfort that can range from mild to debilitating. At Team Rehab UK, we understand how low back pain can impact your quality of life, and we’re here to help you navigate through it with expert care and support.
Understanding Low Back Pain
Low back pain, also known historically as lumbago, can originate from various structures within the back, including muscles, ligaments, nerves, and the spine itself. The pain may be acute, lasting for a few days to a few weeks, or chronic, persisting for more than three months. The lumbosacral region, or lower back, is the most commonly affected area, and the pain can manifest as a dull, constant ache or a sharp, sudden discomfort.
Causes of Low Back Pain
The causes of low back pain are diverse, encompassing both lifestyle factors and underlying medical conditions.
Common causes include:
-
Injury or Trauma: Accidents, falls, or other forms of physical trauma can lead to strains, sprains, or fractures in the lower back.
-
Poor Posture: Prolonged sitting, slouching, or incorrect lifting techniques can place undue stress on the spine, leading to pain.
-
Muscle Tension/Spasms: Overuse or sudden movements can cause muscles in the lower back to tighten or spasm, resulting in pain.
-
Spinal Conditions: Issues such as osteoarthritis, herniated discs, or spinal nerve compression can cause significant discomfort.
-
Degenerative Disc Disease: Over time, the intervertebral discs can degenerate, leading to chronic back pain.
-
Osteoporosis: Weakening of the bones can lead to fractures in the spine, contributing to low back pain.
-
Genetics: Between 2009 and 2014, our founder Chris Heywood was involved with Oxford University studying the role that genetics has to play in the predisposition, onset, and subsequent potential for stem cell treatment of discogenic conditions. Things have moved on in the last decade but even back then we were seeing a clear pattern with up to 70% of intervertebral degeneration being genetically led!
Risk Factors for Low Back Pain
Low back pain is a multifactorial condition, meaning that its onset can be influenced by a combination of various risk factors. Understanding these factors can help in preventing the development of back pain or managing it more effectively. Below is a more detailed exploration of the common risk factors associated with low back pain:
1. Obesity
Impact on the Spine: Carrying excess body weight, particularly in the abdominal area, places additional strain on the spine. This added pressure can lead to changes in the natural curvature of the spine, known as lordosis, which increases stress on the lower back and the facet joints especially.
Mechanical Stress: Obesity can also exacerbate existing back problems by increasing the mechanical load on the spine.
Inflammation: Excess fat tissue in the body is known to produce inflammatory cytokines, which can contribute to chronic inflammation in the body, including the spinal region. It is thought that his inflammation can lead to or worsen existing pain conditions, including low back pain.
2. Age
Degenerative Changes: As we age, the spine undergoes natural degenerative changes. The intervertebral discs, which act as cushions between the vertebrae, lose water content and elasticity, making them less effective at absorbing shocks. This process, known as disc degeneration, can lead to conditions such as herniated discs through subsequent structural weakness, spinal stenosis through reduced space between the vertebra due to loss of disc height, and overloading of structures such as the facet joints, again due to loss of disc height.
As mentioned above, this rate of degeneration, which is linked primarily with age, is also thought to have a significant genetic component as well so you may see common back pain complaints running through your family line.
Joint Wear and Tear: The facet joints in the spine, which help in movement and flexibility, also undergo wear and tear with age (osteoarthritis). This can lead to symptoms characterised by pain, stiffness, and reduced range of motion.
Reduced Bone Density: Aging is also associated with a decrease in bone density, particularly in women after menopause. This can lead to conditions like osteoporosis, which increases the risk of vertebral fractures that can cause significant back pain.
3. Smoking
Impaired Circulation: Smoking has a detrimental effect on circulation, reducing blood flow to the spinal anatomy and tissues. This impaired blood flow can deprive the spinal structures of essential nutrients and oxygen, accelerating degenerative changes in the discs and making them more susceptible to injury and pain.
Degenerative Disc Disease: Studies have shown that smokers are at a higher risk of developing degenerative disc disease. The chemicals in cigarettes can damage the collagen in the discs, making them more prone to degeneration and herniation.
Slowed Healing: Smoking also slows down the body’s natural healing processes. This means that injuries to the spine and back muscles take longer to heal, prolonging pain and discomfort.
4. Sedentary Lifestyle
Muscle Weakness: A lack of regular physical activity can lead to weakened muscles, particularly in the core and back. These muscles play a crucial role in supporting the spine and maintaining proper posture. When they are weak, the spine is more vulnerable to injury and strain, which can lead to chronic low back pain. Please be aware that the term 'core' is not clearly defined so what your personal trainer advised you to work may be very different to the core that your physiotherapist teaches you when you are in pain.
Poor Flexibility: Sedentary behavior, such as prolonged sitting, can also lead to reduced flexibility in the muscles and ligaments of the lower back. This stiffness can limit the range of motion and increase the risk of muscle strains and sprains.
Weight Gain: A sedentary lifestyle is often associated with weight gain, which, as discussed, can place additional strain on the spine and exacerbate low back pain.
5. Medical Conditions
Arthritis: Conditions such as osteoarthritis and rheumatoid arthritis can cause inflammation and damage to the joints in the spine, leading to chronic back pain. Osteoarthritis, in particular, is a degenerative condition that causes the cartilage in the joints to wear down, resulting in pain and stiffness. Rheumatoid arhtritis however is very different, being classes as an auto immune condition that attacks your body with your own immune system
.
Although these get mixed up as they both contain the word arthritis, the way in which we would approach patients care with back pain in these groups may be quite different.
Cancer: Certain types of cancer, such as bone cancer or metastatic cancer that has spread to the spine, can cause severe back pain. Tumors in the spine can compress nerves, leading to pain, numbness, and weakness.
Psychological Factors: Mental health conditions such as anxiety and depression have been shown to be linked to chronic pain, including low back pain. Psychological stress can also cause muscle tension in the back, contributing to pain.
Additionally, individuals with anxiety or depression may have a heightened perception of pain, making it more difficult to manage.
Chronic Conditions: Other chronic conditions, such as fibromyalgia or lupus, can also contribute to ongoing back pain by affecting the muscles, ligaments, and nerves in the back.
6. Pregnancy
Weight Gain: During pregnancy, women naturally gain weight, which can place additional pressure on the lower back. This added weight, particularly in the abdominal area, can shift the center of gravity forward, increasing the curvature of the lower spine (lordosis) and leading to back pain.
Hormonal Changes: Pregnancy causes the release of hormones such as relaxin, which relaxes the ligaments in the pelvis and softens the cervix. While this is essential for childbirth, it can also lead to instability in the spine and pelvis, contributing to lower back pain.
Postural Changes: As the pregnancy progresses, many women adopt compensatory postures to accommodate the growing abdomen. These changes in posture can strain the lower back muscles, leading to pain and discomfort.
Pre-existing Conditions: Women with pre-existing back conditions may find that pregnancy exacerbates their symptoms. The additional strain and postural changes can make managing back pain during pregnancy more challenging.
It is important to seek immediate medical attention if you experience severe symptoms such as loss of bladder or bowel control, numbness around the groin or buttocks, or severe leg weakness, as these could indicate a serious condition like Cauda Equina Syndrome, which requires urgent treatment.
Diagnosis of Low Back Pain
At Team Rehab UK, we begin with a thorough evaluation to diagnose the specific cause of your low back pain. This includes:
Medical History Review: We will discuss your symptoms, lifestyle, medical history, and any previous injuries or treatments. This helps us understand the potential causes of your pain and any underlying conditions that may be contributing to it.
Physical Examination: Our physiotherapists in Northampton will conduct a detailed physical examination to assess your range of motion, muscle strength, reflexes, and sensation. We will also palpate the lower back to identify tender areas and perform specific tests to pinpoint the source of your pain.
Imaging and Diagnostic Tests: While we typically avoid unnecessary imaging, an X-ray or MRI may be recommended in cases where structural abnormalities, such as fractures, scoliosis, or herniated discs, are suspected. An MRI provides detailed images of the soft tissues, including discs, nerves, muscles, and ligaments. In some cases, blood tests may be ordered to diagnose inflammatory conditions like Ankylosing Spondylitis or Rheumatoid Arthritis.
At Team Rehab uk we have some of the most experienced and trusted physiotherapists in the region when it comes to working with spinal pain. Chris worked as a clinical spinal research fellow for 10 years with a leading spinal surgeon, and both Michelle and Anna have both worked closely with complex spinal pain patients as well from the same specialist exposure.
Ensuring Optimal Treatment Outcomes
At Team Rehab UK, we are Northampton based physiotherapists and we emphasise that treatments which are not effective should not continue beyond accepted clinical timescales without further in-depth investigations. We prefer these additional assessments to be conducted by consultant specialists outside our company’s financial interests. This approach ensures we receive an unbiased second opinion, guaranteeing that the care you receive is the most clinically appropriate.
Treatment for Low Back Pain
Most cases of low back pain can be effectively managed with conservative treatments, focusing on reducing pain, restoring mobility, and preventing recurrence. At Team Rehab UK, we offer a range of treatment options tailored to your specific needs:
Physiotherapy
Soft Tissue Release: Techniques such as massage and myofascial release are used to relax tight muscles, improve circulation, and reduce pain.
Bony Mobilisations: Gentle mobilisation techniques help restore normal joint function and reduce stiffness in the spine.
Exercise Prescription: A personalised exercise program designed by our physiotherapists in Northampton focuses on strengthening the core muscles that support the spine, improving flexibility, and promoting overall spinal health. Exercises may include stretching, strengthening, and stabilising activities.
Kinesio Taping: This technique involves applying special tape to support muscles and joints, reduce pain, and improve circulation and mobility.
Electrotherapy: Modalities such as ultrasound, TENS (Transcutaneous Electrical Nerve Stimulation), and interferential therapy can help reduce pain and inflammation in the affected area. We do not use a lot of 'old school' passive electrotherapy at Team Rehab uk, preferring to concentrate more with the hands on and exercise approach.
Acupuncture: An ancient practice that involves inserting thin needles into specific points on the body to alleviate pain and promote healing.
Medication and Other Treatments
Over-the-Counter Pain Relievers: Nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen can help reduce pain and inflammation but should not be taken without professional guidance due other potential side effects in some patient groups, and interaction with other medications.
Heat and Cold Therapy: Applying heat can relax muscles and improve blood flow, while cold packs can reduce inflammation and numb the area to relieve pain.
Activity Modification: Adjusting your daily activities to avoid movements that aggravate your pain is essential. Our team will work with you to identify and modify these activities to reduce strain on your back.
Advanced Treatment Options
If conservative treatments are not providing sufficient relief, or if your condition is more complex, further evaluation and potential referral to a low back specialist in Northampton may be necessary. At Team Rehab UK, we collaborate with top specialists externally to ensure you receive the best, independent, second opinion possible.
Preventing Low Back Pain
Prevention plays a crucial role in managing low back pain and reducing the risk of recurrence. Key strategies include:
Regular Exercise: Engaging in regular physical activity, particularly exercises that strengthen the core muscles, can help support the spine and prevent injury.
Maintain a Healthy Weight: Reducing excess weight can alleviate stress on the lower back.
Good Posture: Practice good posture, especially when sitting for long periods, to reduce strain on your spine.
Quit Smoking: Smoking impairs circulation, which can contribute to spinal degeneration.
Stress Management: Incorporating stress-reducing activities such as yoga or meditation can help prevent tension in the back muscles.
